Apple announced on September 9, 2026, a broad package of health and fitness updates that combines new sensors in Apple Watch with Apple Intelligence-powered analysis capabilities inside the Health app on iPhone and iPad. The company says Apple Watch Series 12 and Apple Watch Ultra 4 will introduce the new Health Sensing System, while the redesigned Health app will arrive later this year on compatible devices.
The change is not limited to adding new metrics; Apple is attempting to turn scattered biometric measurements into daily guidance and a longer-term picture of the user’s health. At the same time, some functions remain tied to specific devices or languages and regions, and Apple emphasizes that Vitals tools are intended for wellness purposes, not medical use.
More Frequent Heart Measurements and a Readiness Score
The Health Sensing System includes new optical and electrical heart sensors, enabling heart-rate measurements every five seconds throughout the day on Apple Watch Series 12 and Apple Watch Ultra 4. The watch also increases the heart-rate variability (HRV) measurement rate to once every five minutes, making measurements 24 times more frequent, according to Apple.
The watch will display two types of HRV: a recovery indicator that helps interpret daily stress and recovery signals, and a general indicator that provides broader context about health status, including cardiovascular health. The nighttime metrics data for Recovery HRV will also include a comparison with the user’s personal baseline, along with the addition of a daytime view that can reveal changes in measurements before they appear overnight.
The two watches also introduce Readiness, a daily score from 0 to 10 based on recent activity, biometric measurements, and sleep results. The score translates into recommendations of recover, take it slower, ready, or go. The score changes throughout the day as new data arrives and explains the factors that influenced it. Apple says the feature’s algorithm was developed using data from the Apple Heart and Movement Study and in collaboration with the company’s exercise scientists and physicians.
Apple’s claim of achieving the highest heart-rate measurement accuracy among wearable devices is based on a company-conducted study involving more than 1,000 participants, with representation from different demographic groups. This result comes from the company’s own source, and the announcement does not provide enough detail to independently evaluate the study’s design or compare it independently.
The Health App Moves from Displaying Data to Interpreting It
With iOS 27 and iPadOS 27, the Health app will gain Insights and Longevity tabs. Insights displays updated summaries throughout the day covering the heart, sleep, readiness, fitness, vital signs, and cycle tracking, with a For You section offering recommendations and articles related to the user’s data.
Longevity brings together long-term data on cardiovascular health, sleep, mental well-being, movement, metabolic health, hearing, and nutrition. It includes Health Age, which compares the trajectory of the user’s metrics with their chronological age based on data such as VO2 max, resting heart rate, sleep, and HRV. Users can add laboratory results such as A1c and LDL to the analysis.
Apple is also adding at-home movement assessments that use the iPhone camera and Apple Watch to measure flexibility, strength, balance, movement mechanics, and VO2 max. The company says processing takes place entirely on the device, without recording, storing, or sharing video, while providing real-time audio and visual feedback using AI vision models.
What Changes in Practice?
With this step, Apple combines continuous sensing, personalized analysis, and tailored health content. For example, the app may display an explanation of an LDL level after a laboratory result appears, or suggest exercises to improve balance after a movement assessment. However, these functions do not turn the watch or app into a substitute for a doctor; the announcement itself connects certain alerts, such as bleeding after menopause or indicators of perimenopause, with speaking to a doctor.
Cycle Tracking in iOS 27 adds support for recording perimenopause and menopause, tracking symptoms and cycles, and notifications when deviations may indicate perimenopause in users aged 40 and older. The updates also include running Workout Buddy on Apple Watch without an iPhone nearby, support for syncing heart rate with compatible fitness equipment through AirPods Pro 3 and iPhone, and improved treadmill readings.
Availability and Limitations
watchOS 27 will become available on September 14 for Apple Watch Series 9 or later, Apple Watch SE 3, and Apple Watch Ultra 2 or later, with an iPhone 11 or later or an iPhone SE 3 running iOS 27 required. The redesigned Health app will arrive later this year, initially in U.S. English, with additional languages to follow.
Laboratory tests covering more than 50 biomarkers will be available for $119 through the app at approximately 2,000 Quest Diagnostics locations in the United States. Health Age requires an Apple Watch, while the VO2 max estimation test requires an Apple Watch, AirPods Pro 3, or an external heart-rate monitor. Apple emphasizes that some features may not be available on all devices, in all regions, or in all languages, and may be affected by local laws.
