Moderna and Merck's Personalized Cancer Vaccine Reduces Risk of Death and Melanoma Recurrence by 42%

Moderna and Merck have announced promising results from the Phase 3 clinical trial of their personalized mRNA-based vaccine for the treatment of melanoma, the deadliest form of skin cancer. The results showed that the combination of the vaccine and standard immunotherapy reduced the risk of death or disease recurrence by 42% compared to standard treatment alone.

Mechanism of Action and Technology Used

The vaccine, called intismeran (or mRNA-4157), is based on synthetic mRNA technology that carries the unique genetic code for up to 34 mutations in each patient's specific cancer cells. To produce this personalized vaccine, researchers compare the genetic sequences of a patient's cancer cells with their healthy cells to identify cancer-specific mutations that can help the immune system precisely target malignant cells.

Clinical Trial Results

The study included 1,137 patients who suffered from advanced stages of melanoma (from stage IIB to IV) after undergoing surgical resection. Participants were randomly assigned to receive a combination of the vaccine and Keytruda (Merck's monoclonal antibody therapy), or to receive Keytruda with a placebo.

  • Recurrence-Free Survival (RFS): After two years, the percentage of patients who remained alive without cancer recurrence was 82% in the vaccine group, compared to 70% in the standard treatment group.
  • Distant Metastasis-Free Survival (DMFS): Vaccine treatment reduced the risk of distant metastasis or death by 38%, with 88.5% of patients in the vaccine group remaining free of distant metastasis compared to 80% in the control group.

Why This News Matters

These results are a milestone in the field of medical technology, confirming the possibility of developing cancer treatments tailored to each patient based on their genetic profile. Although the therapeutic effect was slightly lower than that observed in previous Phase 2 trials, researchers view this study as the first Phase 3 trial to demonstrate a tangible clinical improvement that exceeds current standards for melanoma treatment, paving the way for regulatory approval applications.
